- Account Balance Worksheet - to help you balance and prepare to close your old account.
- Account Closure Request - to give to your old bank so that they can prepare to close your accounts
- Deposit & Payment Checklist - a checklist to make certain all of your direct deposit and automatic payments are redirected to your new UCB account
- Direct Deposit Change Form - to give to your employer to set up your new payroll direct deposit
- Automatic Debit Change Form - to give to each company you have setup for automatic payments
